Adult Dosing
Opioid-induced constipation
- < 38 kg (<84 lbs): 0.15 mg/kg SC every other day; Max: 1 dose/24 hrs
- 38 to < 62 kgs (84 to < 136 lbs): 8 mg SC every other day; Max: 1 dose/24 hrs
- 62 to 114 kgs (136 to 251 lbs): 12 mg SC every other day; Max: 1 dose/24 hrs
- >114 kgs (>251 lbs): 0.15 mg/kg SC every other day; Max: 1 dose/24 hrs
Pediatric Dosing

Renal Dose Adjustment (Based on CrCl )
- Mild to moderate impairment: No dose adjustments
- Severe impairment (CrCl < 30 mL/min): Decrease dose by 50%
Hepatic Dose Adjustment
- Mild to moderate impairment (Child Pugh Class A/B): No dose adjustments
- Severe impairment (Child Pugh Class C): Dose adjustments not defined

- Discontinue the therapy if severe of persistence diarrhea occurs during treatment
- GI perforation associated with localized or diffuse reduction of structural integrity in the wall of the GI tract has been reported in advanced illness patients. Use cautiously in patients with known or suspected lesions of the GI tract
- Discontinue therapy and inform physician if they develop severe, persistent, and/or worsening abdominal symptoms
