Erectile dysfunction

• Start 125 mcg or 250 mcg intraurethral, titrate the dose in a stepwise manner until an erection sufficient for sexual intercourse is achieved

Note:

• There should be at least 12-hr gap between doses
• Use lowest effective dose

• Hypersensitivity to any of the ingredients of the product
• Abnormal penile anatomy
• Penile implants
• In patients who are prone to venous thrombosis (e.g. sickle cell anemia or trait, thrombocythemia, leukemia, polycythemia, multiple myeloma) or who have a hyperviscosity syndrome that might predispose them to priapism
• Men for whom sexual activity is not advisable
• Sexual intercourse with a pregnant woman unless the couple uses a condom barrier

• Drug titration should be done under medical supervision as it could result in symptomatic hypotension and syncope
• Caution patients against driving or performing hazardous tasks because alprostadil may cause syncope or hypotension
• Prior to therapy initiation, perform a complete medical history and physical examination to exclude reversible causes of erectile dysfunction
• Use lowest effective dose and monitor for symptoms of hypotension
• Therapy may cause urethral abrasion resulting in minor bleeding or spotting if improperly administered; use cautiously and consider the risk/benefit ratio in these patients prior to prescribing
• Examine the cardiac fitness of patients prior to treating erectile dysfunction because alprostadil increases heart rate as well as cardiac work
• Prolonged erection and priapism have been reported and these events can cause penile injury. In such cases, patients should lower the dose or discontinue the treatment

Cautions: Use cautiously in

• Patients on anticoagulant therapy or bleeding disorders
• Pregnant sexual partner
• Concomitant use of anti-hypertensive medications

Supplemental Patient Information

• Inform patients and their partners that alprostadil does not offer any protection from the transmission of sexually transmitted diseases
• Advise patients and their partners to undergo counseling regarding the protective measures required to guard against the spread of sexually transmitted agents, including HIV
• Encourage partners with extended period of sexual abstinence to seek medical advice prior to resuming sexual intercourse
• Instruct couples to use adequate contraception if the female partner is of childbearing potential
