Adult Dosing
Symptoms of bladder instability [TABS]
- 5 mg PO tid; Max: 5 mg qid
- Elderly: 2.5 PO mg bid-tid
Overactive bladder [ETABS]
- Starting dose: 5-10 mg PO Daily at same time each day
- May increase by 5 mg qwk to achieve a balance of efficacy and tolerability
- Max: 30 mg/day
Note:
- Do not chew, divide, or crush the ETABS
Pediatric Dosing
Age 5 or older Symptoms of bladder instability [TABS]
- 5 mg PO bid; Max: 5 mg tid
Age 6 or older detrusor overactivity [ETABS]
- 5 mg PO daily at same time each day
- May increase by 5 mg qwk to achieve a balance of efficacy and tolerability
- Max: 20 mg/day

- Oxybutynin can cause a variety of CNS anticholinergic effects such as hallucinations, agitation, confusion and somnolence. Monitor the patient for signs of anticholinergic CNS effects, particularly in the first few months after starting treatment or increasing the dose. Discontinue the drug or reduce the dose if a patient experiences anticholinergic CNS effects
- Patients with preexisting dementia treated with cholinesterase inhibitors are at risk of aggravation of symptoms, hence use cautiously in these patients
- Use of oxybutynin can aggravate the symptoms of hyperthyroidism, coronary heart disease, congestive heart failure, cardiac arrhythmias, hiatal hernia, tachycardia, hypertension, myasthenia gravis, and prostatic hypertrophy
- Use of oxybutynin in patients with ulcerative colitis may suppress intestinal motility to the point of producing a paralytic ileus and precipitate or aggravate toxic megacolon, a serious complication of the disease
- Use cautiously in patients with clinically significant bladder outflow obstruction because of the risk of urinary retention
- Angioedema has been reported with the first doses requiring hospitalization and emergency treatment

- Bladder outflow obstruction
- Gastrointestinal obstructive disorders
- Ulcerative colitis
- Intestinal atony
- Gastroesophageal reflux
- Concomitant use with drugs such as bisphosphonates that causes or exacerbates esophagitis
- Concomitant use with other anticholinergic agents
Supplemental Patient Information
- Advise patients to avoid becoming overheated or dehydrated during exercise and in hot weather as oxybutynin can cause heat prostration in the presence of high environmental temperature
- Oxybutynin can produce drowsiness (somnolence) or blurred vision, advise patients to avoid engaging in potentially dangerous tasks like operating machines or driving vehicle
- Advise patients to abstain from alcohol during therapy

Breastfeeding: Safety unknown; Literature unavailable on the use of oxybutynin during breastfeeding. Prolonged use of oxybutynin reduces milk production or milk letdown, but a single dose is unlikely to interfere with breastfeeding.
