Adult Dosing

Insomnia

• Sleep onset: 2 mg PO qhs. May increase to 3 mg PO qhs
• Elderly or debilitated patients: Start 1 mg PO qhs (Max: 2 mg/day)
• Note: Avoid administration with high fat meals as it may interfere with absorption

• Safety and effectiveness in patients < 18 years has not been established

Renal Dose Adjustment

• Renal impairment: No dose adjustment

Hepatic Dose Adjustment

• Mild to moderate hepatic impairment: Use with caution
• Severe hepatic impairment: Start 1 mg PO qhs. Max: 2 mg/day

• Failure of remission of insomnia after 7- 10 days may indicate the presence of a primary psychiatric and/or medical illness. Re-evaluate the diagnosis in such cases
• Due to rapid onset of action, eszopiclone should only be ingested immediately prior to going to bed or after the patient has gone to bed and has experienced difficulty falling asleep
• Eszopiclone is associated with risk of complex sleep-related behaviors such as sleep-driving, making phone calls, or preparing and eating food while asleep
• Monitor for suicidal thoughts and actions (including completed suicides)
• Dose adjustment may be necessary when eszopiclone is administered with other CNS-depressant agents, due to additive effects

Cautions: Use cautiously in

• Hepatic impairment
• CNS depressant
• Geriatric population (use lower initial dose)
• Debilitated patients
• Impaired respiratory function
• Administration of CYP3A4 inhibitors (use lower initial dose)
• History/signs of depression
• History of psychiatric disorder

• CNS: Headache, anxiety, confusion, depression, dzziness, hallucinations, libido decreased, nervousness, somnolence, abnormal dreams, neuralgia, suicidal ideation, amnesia
• RESP: Infection
• MISC: Viral Infection, accidental Injury, pain, anaphylactic reaction
• GI: Dry Mouth, dyspepsia, nausea, vomiting, unpleasant Taste, diarrhea
• DERM: Rash, pruritus
• GU: Dysmenorrhea, gynecomastia, UTI

• Pyrrolopyrazine; exact mechanism of action is unknown; effect may result from interaction with GABA-receptor complexes at binding domains located close to or allosterically coupled with benzodiazepine receptors
• Eszopiclone is rapidly absorbed after oral administration
• Extensively metabolized in liver by CYP450 (3A4 and 2E1 substrate)
• Renally excreted: Approximately 75% (10 % unchanged)
• Half-Life: 6 hrs
