Adult Dosing
Hypertension
- Start 150 mg PO qd if monotherapy, 75 mg PO qd if also on diuretic
- MAX: 300 mg/day
Heart failure (Non-FDA Approved)
- Note that similar dosing to that used for hypertension is typically used for this indication
Diabetic nephropathy in type 2 diabetes mellitus
Pediatric Dosing
- Safety and efficacy in pediatric population have not been established

- Irbesartan can cause fetal/neonatal morbidity/mortality if used during pregnancy. Discontinue as soon as possible once pregnancy is detected. Drugs that act on the renin-angiotensin system used during the second and third trimesters has been associated with fetal and neonatal injury and death. If oligohydramnios is observed, discontinue unless it is considered life saving to the mother
- Correct volume or salt depletion before starting therapy with irbesartan. Symptomatic hypotension may occur in patients with intravascular volume- or sodium-depletion, e.g., patients treated with diuretics or patients on dialysis
- In patients whose renal function may depend on the activity of the renin-angiotensin-aldosterone system (eg, severe CHF), use may be associated with oliguria, progressive azotemia, acute renal failure, and death
- Coadministration of NSAIDs, including selective COX-2 inhibitors with angiotensin II receptor antagonists, in patients who are elderly, volume-depleted or with compromised renal function may lead to deterioration of renal function including acute renal failure. Monitor renal function periodically in patients patients receiving irbesartan and NSAID therapy

- Renal artery stenosis
- Hepatic impairment
- Volume- or salt-depleted patients or patients receiving large doses of diuretics
- Black patients (efficacy decreased)
- Women of childbearing potential
- Patients with primary renal disease or heart failure
- Concomitant use of NSAIDs
