Adult Dosing
Management of renal tubular acidosis with calcium stones, hypocitraturic calcium oxalate nephrolithiasis, uric acid lithiasis with or without calcium stones
Severe Hypocitraturia (Urinary Citrate <150 mg/day)
- Initial: 60 mEq/day, i.e. 30 mEq bid or 20mEq tid with meals or within 30 minutes after meals or bedtime snack
- Max: 100 mEq per day
Mild to Moderate Hypocitraturia (Urinary Citrate >150 mg/day)
- Initial: 30 mEq per day, i.e. 15 mEq bid or 10 mEq tid with meals or within 30 minutes after meals or bedtime snack
- Max: 100 mEq per day
Pediatric Dosing
- Safety and effectiveness in pediatric patients have not been established

- Potassium citrate may produce hyperkalemia and cardiac arrest in patients with impaired mechanisms for excreting potassium
- Use of potassium citrate should be avoided in chronic renal failure, or any other condition which impairs potassium excretion such as severe myocardial damage or heart failure
- Carefully monitor for signs of hyperkalemia with regular blood tests and ECGs
- Upper gastrointestinal mucosal lesions have been reported with use of potassium citrate. If severe vomiting, abdominal pain or gastrointestinal bleeding is observed, discontinue treatment promptly and investigate the possibility of bowel perforation or obstruction

- Recent myocardial infarction
- Congestive heart failure
Pregnancy Category:C
Breastfeeding: Safety unknown. As per manufacturer, nursing mothers should receive the drug only of clearly required.
