Adult Dosing
Osteoarthritis
- 200 mg PO qd. Alt: 100 mg PO bid
- MAX: 400 mg/day
Rheumatoid arthritis
- 100-200 mg PO bid
- MAX: 400 mg/day
Ankylosing Spondylitis
- 200 mg PO daily. Alt: 100 mg PO bid
- After 6 wks may increase to 400 mg/day (single or divided doses)
- If no effect on 400 mg/day x 6 wks, consider alternate treatment
- MAX: 400 mg/day
Colorectal polyps (familial adenomatous polyposis)
Dysmenorrhea
- Start 400 mg as single dose followed by 200 mg if necessary on day 1; thereafter 200 mg bid
Acute pain
- Start 400 mg as single dose followed by 200 mg if necessary on day 1; thereafter 200 mg bid
Note: Start at 50% of lowest dose if poor CYP2C9 metabolizer. Use lowest dose; shortest treatment duration.
Pediatric Dosing
Juvenile Rheumatoid arthritis
- > 2 yrs; 10-25 kg: 50 mg PO bid
- > 2 yrs, > 25 kgs: 100 mg PO bid
Note: Consider alternative treatment if poor CY92C9 metabolizer.

- Chronic use may cause an increased risk of serious cardiovascular thrombotic events, MI and stroke
- NSAIDs, including celecoxib can cause fatal gastrointestinal events including bleeding, ulceration, and perforation of the stomach, small intestine or large intestine
- Celecoxib may lead to onset of new hypertension or cause worsening of pre-existing hypertension. Monitor Blood Pressure closely during treatment
- It may cause fluid retention and edema in some patients
- Elevated liver enzymes may occur. Rare cases of severe hepatic reactions (eg, jaundice, hepatic failure) have occurred. Discontinue use if liver disease develops or if systematic manifestations occur (eg, eosinophilia, rash)
- Long-term use of NSAIDs has resulted in renal papillary necrosis

- Renal impairment
- Hepatic impairment
- Cardiovascular disease
- Presence of cardiac risk factors
- Use with warfarin, lithium (decrease Li levels), ACE and angiotensin II antagonists (decrease effect)
- Hypertension
- CHF
- Fluid retention
- Hx of PUD/GI bleed
- Hematological disorders
- Dehydration
- Asthma
- Smoker
- Poor CYP2C9 metabolizer
- Elderly patients/debilitated patients
- Long term use
- Diuretic use
- Pregnancy
Pregnancy Category:C. Pregnancy category D from 30 weeks of gestation onward.
Breastfeeding: Excreted in breastmilk in low levels. Amounts ingested by the infant would be small and would not be expected to cause any adverse effects in breastfed infants.
