- Start 200 mg/kg/day IV in 4 divided doses over 20–40 minutes; max: 7 days

Renal Dose Adjustment: (Based on CrCl)
- 20-40 mL/min: 12.5-37.5 mg/kg q12 hrs
- 10-20 mL/min: 12.5-37.5 mg/kg q24 hrs
- <10 mL/min: 12.5-37.5 mg/kg q24-48 hrs
Hepatic Dose Adjustment
- Hepatic impairment: Dose adjustments not defined
- Risk of accumulation in renal impairment; monitor hematologic, renal, and hepatic status of all patients before initiating therapy with flucytosine [US Black Box Warning]
- Monitor serum electrolytes, hematological status, renal and hepatic status (alkaline phosphatase, SGOT& SGPT) at baseline and at periodic intervals
Cautions: Use cautiously in
- Renal impairment (may cause accumulation of the drug)
- Bone marrow depression
- Hematological disease
- Radiation therapy
- Elderly patient
