Actonel
Treatment of postmenopausal osteoporosis
- 5 mg PO qd
- Alt: 35 mg PO qwk, or 75 mg on 2 consecutive days each month, or 150 mg qmo
Prevention of postmenopausal osteoporosis
- 5 mg PO qd
- Alt: 35 mg PO qwk
Glucocorticoid induced osteoporosis
Osteoporosis, male
Paget's disease
- 30 mg PO qd for 2 mo
- May repeat after 2 months post treatment observation period

Notes- Give with water >30min before first meal, remain upright for 30 mins
- Calcium and vitamin D supplementation recommended if inadequate dietary intake
Pediatric Dosing

Renal Dose Adjustment (Based on CrCl)
- > 30 mL/min: No dose adjustments
- <30 mL/min: Use not recommended
Hepatic Dose Adjustment
- Hepatic impairment: No dose adjustments
- Severe irritation of the upper gastrointestinal mucosa can occur. Follow dosing instructions carefully. Discontinue therapy if patients develop dysphagia, odynophagia, retrosternal pain or new or worsening heartburn
- Hypocalcemia and other disturbances of bone and mineral metabolism must be corrected prior to use
- Osteonecrosis of the jaw has been reported rarely. Risk factors for osteonecrosis of the jaw include invasive dental procedures, diagnosis of cancer, concomitant therapies (e.g., chemotherapy, corticosteroids), poor oral hygiene, and co-morbid disorders
- Severe bone, joint, or muscle pain may occur. Severe and occasionally incapacitating pain has been reported in postmarketing experience. Discontinue use if severe symptoms develop
- Before initiating therapy in patients with glucocorticoid-induced osteoporosis, sex steroid hormonal status of both men and women should be ascertained and appropriate therapy considered
- Bisphosphonates may interfere with bone-imaging agents
- Should be taken on empty stomach with at least 6 ounces of water, at least 30 min before first food, beverage or medication of the day. Maintain upright position for 30 mins after taking medication

- Active upper GI problems (Barett's esophagus, dysphagia, gastritis, duodenitis)
- Dental procedures (invasive)
- Concomitant chemotherapy
- Concomitant corticosteroids
- Anemia
- Coagulopathy
- Infection
Pregnancy Category:C
Breastfeeding: Safety unknown. No published experience exists with risedronate during breastfeeding. An alternate drug may be preferred, especially while nursing a newborn or preterm infant.
