Adult Dosing

Type 2 diabetes mellitus

• 5 mg PO qd

• When co-administered with an insulin secretagogue such as a sulfonylurea, a lower dosage of insulin secretagogue must be considered to reduce the risk of hypoglycemia
• Dose can be taken with or without food

• Linagliptin should not be used in patients with type 1 diabetes or for the treatment of diabetic ketoacidosis

• Postmarketing cases of acute pancreatitis, with fatalities have been reported in patients receiving linagliptin. Discontinue therapy and initiate appropriate management upon suspicion of pancreatitis
• When used concomitantly with insulin secretagogues, administer a lower dose of the insulin secretagogue to reduce the risk of hypoglycemia
• Safety and effectiveness for reducing macrovascular risk have not been established with linagliptin or any other antidiabetic drug
• Periodically monitor blood glucose and A1C levels during therapy

• Hypoglycemic agent; inhibits dipeptidyl peptidase-4 (DPP-4), an enzyme which degrades the incretin hormones, glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP), resulting in increased levels of active incretin hormones. It stimulates the release of insulin in a glucose-dependent manner and decreases the glucagon levels in the circulation
• Absolute bioavailability is 30%
• Metabolized minimally in the liver; CYP450: 3A4 substrate
• Excreted in feces (80%) and urine (5%)
