Upper respiratory tract infections
Pharyngitis/Tonsillitis
- 500 mg PO q24 hrs x 10 days
Acute sinusitis
- Mild: 250 mg PO q12 hrs x 10 days
- Mod to Severe: 500 mg PO q12 hrs x 10 days
Lower respiratory tract infections
- 500 mg PO q12 hrs x 10 days
Skin/Skin structure infections, umcomplicted bacterial
- 250-500 mg PO q12 hrs x 10 days
- Alt: 500 mg PO q24 hrs x 10 days
Pediatric Dosing
Otitis media
Child (6 months-12 yrs)
- 30 mg/kg/day PO divided q12 hrs x 10 days
- Max: 1000 mg/day
Acute sinusitis
Child (6 months-12 yrs)
- 15-30 mg/kg/day PO divided q12 hrs x 10 days
- Max: 1000 mg/day
Pharyngitis/tonsillitis
Child (2-12 yrs)
- 15 mg/kg/day PO divided q12 hrs x 10 days

Skin/Skin structure infections, umcomplicted bacterial

Renal Dose Adjustment (Based on CrCl)
- > 30 mL/min: Give usual dose
- < 30 mL/min: Reduce dose by 50%
- HD: Administer after the hemodialysis session
Hepatic Dose Adjustment
- Hepatic impairment: No dose adjustments
- Before starting therapy with cefproxil, carefully inquire if patients have had previous hypersensitivity to cephalosporins, penicillins or other b-lactams. Extreme caution must be exercised as cross reactivity among beta-lactam antibiotics is clearly documented and may occur in up to 10% of patients
- If an allergic reaction occurs, discontinue use and institute appropriate therapeutic measures
- Cefproxil should be used to treat infections that are proven or strongly suspected to be caused by susceptible bacteria. Prescribing in the absence of a proven or strongly suspected bacterial infection increases the risk of the development of drug-resistant bacteria
- Obtain culture and susceptibility tests before starting therapy to determine the susceptibility of the causative organisms
- Clostridium difficile associated diarrhea which may vary in severity from mild to fatal colitis, has been reported with use of cephalosporins
- Prolonged use may result in the overgrowth of nonsusceptible organisms

- Hypersensitivity to penicillins
- Hx of GI disease
- Seizure disorder
