- Cases of increased serum transaminase levels, jaundice, hepatic encephalopathy, and death due to acute hepatic failure have been reported. In some patients, hepatic injury was reversed after discontinuation of therapy. About 50% of the cases occurred within the first 3 months of treatment [US Black Box Warning]
- Monitor serum transaminase levels prior to starting treatment, then monthly for the first 4 months of treatment, and periodically thereafter. Avoid use in patients whose ALT levels >2x the ULN. Perform LFTs at the first signs and symptoms suggestive of liver dysfunction and immediately discontinue the therapy at any time a patient has jaundice or if the ALT >2x the ULN, and closely follow-up LFTs until resolution [US Black Box Warning]
- It is indicated for men only. Not recommended in women, and should not be used particularly for non-serious or non-fatal situations in this population
- It may cause fetal harm when administered to a pregnant woman
- A metabolite of flutamide, 4-nitro-3-fluoro-methylaniline, may cause several toxicities such as methemoglobinemia, hemolytic anemia and cholestatic jaundice. Monitor methemoglobin levels in patients susceptible to aniline toxicity including individuals with glucose-6-phosphate dehydrogenase deficiency, hemoglobin M disease, and smokers
- Gynecomastia may occur in patients receiving flutamide together with medical castration
- Monitor serum PSA levels regularly to assess patients response to therapy. If PSA levels rise significantly during therapy, consider evaluating the patient for clinical progression. Consider temporary discontinuation of flutamide while continuing the LHRH for patients with objective progression of disease along with an elevated PSA
- Increased prothrombin time have been reported in patients receiving prolonged warfarin therapy after treatment initiation with flutamide
Supplemental Patient Information
- Inform patients that flutamide capsules should be co-administered with the drug used for medical castration and that they should not interrupt their dosing or stop these medications without consulting their physician
