Adult Dosing
Advanced breast cancer in postmenopausal women
Treatment of postmenopausal women who have received 2-3 yrs of tamoxifen therapy
- After 2-3 yrs of tamoxifen treatment, continue exemestane 25 mg PO qd after meal, in the absence of recurrence or contralateral invasive breast cancer, until completion of a total of five consecutive years of adjuvant hormonal therapy
Note:
- Patients receiving exemestane with a potent CYP3A4 inducer (rifampicin, phenytoin): give 50 mg PO qd after a meal
Pediatric Dosing
- Safety and effectiveness in pediatric patients have not been established

- Therapy may cause fetal harm in pregnant women and is not recommended in pregnant women and those planning to become pregnant
- Avoid co-administration with estrogen-containing agents as these could interfere with its pharmacologic action
- Hematological abnormalities such as lymphocytopenia may occur in patients with advanced breast cancer
- Patients with advanced breast cancer have been reported with increase in serum levels of AST, ALT, alkaline phosphatase, and gamma glutamyl transferase, but appear mostly attributable to the underlying presence of liver and/or bone metastases
- Routine assessment of 25-hydroxy vitamin D levels should be done before starting the therapy due to the high prevalence of vitamin D deficiency in women with early breast cancer. Women with vitamin D deficiency should receive vitamin D supplementation
- Reductions in bone mineral density may be seen with prolonged therapy
