Adult Dosing
Neuropathic Pain associated with diabetic peripheral neuropathy
- Start 50 mg PO tid; may gradually be increased to 100 mg tid within 1 wk, based on efficacy and tolerability
- Max: 300 mg/day
Post-herpetic neuralgia
- Start 75 mg PO bid or 50 mg PO tid; may gradually be increased to 300 mg/day (divided bid-tid) within 1 wk, based on efficacy and tolerability. If inadequate relief after 2-4 wks at doses of 300 mg/day, may increase to 300 mg bid or 200 mg tid
- Max: 600 mg/day
Partial seizures
- Start: 50 mg PO tid or 75 mg PO bid
- Max: 600 mg/day
Fibromyalgia
- Start 75 mg PO bid, may be increased to 150 mg PO bid within 1 wk based on efficacy and tolerability
- Max: 450 mg/day
Neuropathic pain associated with spinal cord injury
- 75 mg PO bid, may be increased to 150 mg PO bid within 1 wk based on efficacy and tolerability
- Max: 600 mg/day
Note: Withdraw gradually (over at least 1 wk) to minimize potential of increased seizure frequency
Pediatric Dosing
- Safety and efficacy in pediatric patients (<18 years of age) have not been established

Renal Dose Adjustment (Based on CrCl)
- 60 mL/min: 150-600 mg/day PO divided bid-tid
- 30-60 mL/min: 75-300 mg/day PO divided bid-tid
- 15-30 mL/min: 25-150 mg/day PO divided qd-bid
- <15 mL/min: 25-75 mg/day PO qd
- Hemodialysis:
- If on 25 mg qd regimen: Give 25 or 50 mg as supplement
- If on 25-50 mg qd regimen: Give 50 or 75 mg as supplement
- If on 50-75 mg qd regimen: Give 75 or 100 mg as supplement
- If on 75 mg qd regimen: Give 100 or 150 mg as supplement
Hepatic Dose Adjustment
- Hepatic impairment: No dose adjustments
- Antiepileptic drugs, including pregabalin, increase the risk of suicidal thoughts or behavior. Monitor patients for the emergence or worsening of depression, suicidal thoughts or any unusual changes in behavior
- Life-threatening angioedema with respiratory compromise requiring emergency treatment, has occurred during postmarketing surveillance. Discontinue drug immediately in such instances
- Hypersensitivity reactions (e.g. hives, dyspnea, and wheezing) can occur; discontinue therapy immediately
- Creatinine kinase elevations may occur. Discontinue therapy if myopathy is diagnosed or suspected or if markedly elevated creatinine kinase levels occur
- Risk of increased seizure frequency in patients with seizure disorders upon abrupt wihdrawl; taper over at least 1 wk) to discontinue
- Pregabalin may cause Dizziness and Somnolence and impair patients ability to drive or operate machinery
- Peripheral edema may occur in patients taking pregabalin and a thiazolidinedione antidiabetic agent. Use caution with coadministration
- Ophthalmologic effects viz reduction in visual acuity, visual field changes, and funduscopic changes have been reported
Cautions: Use cautiously in
- Renal impairment (refer dose adjustment section)
- Risk factors/history of angioedema
- Hx of depression
- CHF (NYHA class III, IV )
- Diabetes mellitus
- Fluid retention
- Thiazolidinedione antidiabetic agent use
- Lactation
- Geriatric population
- Avoid abrupt withdrawal (taper over at least 1 week)
Pregnancy Category:C
Breastfeeding: Safety unknown; is excreted in breast milk of lactating rats. Due to the potential for tumorigenecity, manufacturer advises discontinuation of nursing or discontinuation of drug administration (taking into consideration the importance of the drug to the mother).
