Adult dosing
Acute alcohol withdrawal
- Day 1:
- 30 mg PO initially, then 30-60 mg in divided doses
- Max: 90 mg/day
- Day 2:
- 45-90 mg PO in divided doses
- Day 3:
- 22.5-45 mg PO in divided doses
- Day 4:
- 15-30 mg PO in divided doses; then gradually reduce dose to 7.5-15 mg/day PO and continue until patient is stable
Anxiety
- 15-60 mg/d PO in divided doses or in single dose at bedtime
- Initial bedtime dose should not exceed 15 mg, but 11.25 mg and 22.5 mg single-dose tablets may be used when patient is stable on equivalent divided doses
- Usual: 30 mg/day PO
- In elderly or debilitated patients, initiate with 7.5-15 mg/day in divided doses or as a single dose at bedtime
Adjunct in partial seizures
- Initial: Up to 7.5 mg PO tid; titrate: increase by up to 7.5 mg/wk
- Max: 90 mg/day

- Clorazepate is not recommended for use in depressive neuroses or in psychotic reactions
- Monitor the patient receiving any antiepileptic drug including clorazepate for the signs of depression, suicidal thoughts or behavior, and/or any unusual changes in mood or behavior, as antiepileptic drugs increase the risk of suicidal tendencies in the patients
- May cause CNS depression, which may impair physical or mental abilities; patients must be cautioned against performing tasks which require mental alertness such as operating dangerous machinery including motor vehicles
- Avoid abrupt withdrawal as there is a possibility of seizures getting worse
- Monitor blood counts, hepatic function, respiratory and cardiovascular status
- Use clorazepate cautiously in elderly and debilitated patients and increase dosage gradually
- Abrupt cessation of clorazepate may be associated with withdrawal symptoms

- Drug or alcohol abuse
- Suicidal tendencies
- Renal disease
- Hepatic disease
- Change dose gradually
- Therapy for >4 months
- Pulmonary impairment
- Sleep apnea
- Seizure history
- Elderly or debilitated patients (start lower dose)
Supplemental Patient Information
- Inform patients, their caregivers, and families that antiepileptic drugs (AEDs) may increase the risk of suicidal thoughts and behavior; be alert for the emergence/worsening of the signs and symptoms of depression, any unusual changes in mood or behavior, or the emergence of suicidal thoughts, behavior, or thoughts about self-harm
- Advise caregivers and their families to report any behavior changes immediately to healthcare providers
- Advise patient to inform physician if she becomes pregnant during the therapy or plans to become pregnant
- Take physician's opinion before increasing or abruptly discontinuing the drug as clorazepate may produce psychological and physical dependence
