- 10 mcg INH q12 hrs (Note: approximately 8.5 mcg of formoterol fumarate is delivered by each actuation of Foradil Certihaler, corresponding to a metered dose of 10 mcg of formoterol fumarate)
Exercise-induced bronchospasm

- Increased risk of asthma related death; insufficient data to determine whether concomitant use of inhaled corticosteriods or other long term asthma control medications mitigate increased risk of asthma-related death [US Black Box Warning]
- Do not exceed recommended dosage
- Use only in combination with long-term asthma control medication e.g. corticosteroids. Do not use if asthma is adequately controlled on low or medium dose inhaled corticosteroids
- Long acting beta2 agonists increase the risk of asthma-related hospitalization in adolescent and pediatric patients; use fixed dose combination of an inhaled corticosteroid and long acting beta2 agonists
- Risk of higher incidence of serious asthma exacerbations vs. placebo; do not initiate in patients with significantly worsening or acutely deteriorating asthma
- Do not use in conjunction with an inhaled, long-acting beta2-agonist
- It is not a substitute for inhaled or oral corticosteroids; do not stop or reduce dosage of corticosteroids at the of starting therapy with formoterol
- Instruct patients who have been taking inhaled, short-acting beta2-agonists on a regular basis to discontinue and use those drugs only for symptomatic relief of acute symptoms
- Can produce life-threatening paradoxical bronchospasm
- Hypersensitivity reactions like anaphylactic reactions, angioedema, bronchospasm, rash, and urticaria may occur
- Clinically significant changes in SBP and/or DBP pulse rate and electrocardiograms have occurred

- Hypersensitivity to milk proteins
- Cardiovascular disorders
- Coronary insufficiency
- Cardiac arrhythmias
- Hypertension
- Diabetes
- Convulsive disorders
- Thyrotoxicosis
- Thyroid disorder
- Hypokalemia
