Adult Dosing

Tooth extraction, hemophilia

• Before tooth extraction: 10 mg/kg IV
• Following tooth extraction: 10 mg/kg IV q6-8 hrs x 2-8 days

Pediatric Dosing

Tooth extraction, hemophilia

• Before tooth extraction: 10 mg/kg IV
• Following tooth extraction: 10 mg/kg IV q6-8 hrs x 2-8 days

• Indicated in patients with hemophilia for short-term use (two to eight days) to reduce or prevent hemorrhage and reduce the need for replacement therapy during and following tooth extraction

• Hypersensitivity to any of the ingredient of the product
• Acquired defective color vision
• Subarachnoid hemorrhage
• Active intravascular clotting

Renal Dose Adjustment (Based on serum creatinine)

• 1.36-2.83 mg/dL: 10 mg/kg IV q12 hrs
• 2.83-5.66 mg/dL: 10 mg/kg IV q24 hrs
• >5.66 mg/dL: 10 mg/kg IV q48 hrs or 5 mg/kg IV q24 hrs

Hepatic Dose Adjustment

• Hepatic impairment: Dose adjustments not defined

• Cerebral edema or infarction may occur in patients with subarachnoid hemorrhage
• Execute ophthalmological examination, including visual acuity, color vision, eye-ground and visual fields at regular interval in patients who are on treatment for longer period and discontinue the treatment if any changes are found
• Ureteral obstruction, venous and arterial thrombosis or thromboembolism has been reported in patients treated with tranexamic acid
• Tranexamic acid should not be administered concomitantly with Factor IX Complex concentrates or Anti-inhibitor Coagulant concentrates as the risk of thrombosis may be increased
• Patients with disseminated intravascular coagulation (DIC) under tranexamic acid treatment should be supervised strictly by physician

Cautions: Use cautiously in

• Renal impairment
• Hematuria originating in the upper urinary tract
• Hx of thromboembolism
• DIC
• Elderly

Pregnancy Category:B

Breastfeeding: Safety unknown, Tranexamic acid is present in the mother’s milk at a concentration of about one hundredth of the corresponding serum concentrations hence used during lactation only if clearly needed. Manufacturer advises caution.

• EENT: Visual abnormalities, retinal degeneration, retinal artery/vein thrombosis
• CV: Hypotension
• HEMA: Anemia, thromboembolic disorders
• GI: Abdominal pain, diarrhea, nausea, vomiting
• GU: Ureteral obstruction
• RENAL: Cortical necrosis

• Antifibrinolytic agents; reduce conversion of plasminogen to plasmin (fibrinolysin) by inhibiting activation of plasminogen
• Minimally metabolized by liver; CYP450: unknown
• Renally excreted: >95% (unchanged)
• Half-life: 2 hrs
