• Modafinil is indicated to improve wakefulness in adult patients with excessive daytime drowsiness associated with narcolepsy, obstructive sleep apnea/hypopnea syndrome, and shift work sleep disorder
• In obstructive sleep apnea, it is indicated as an adjunct to standard treatment for the underlying obstruction

• Patients with known hypersensitivity to modafinil, armodafinil or its inactive ingredients

Renal Dose Adjustment

• Severe impairment: use with caution

Hepatic Dose Adjustment

• Severe hepatic impairment: 100 mg PO qam

• Serious rash requiring hospitalization and discontinuation of treatment has been reported in adults and children in association with the use of modafinil. Rare cases of serious or life-threatening rash, including Stevens-Johnson syndrome, Toxic Epidermal Necrolysis and drug Rash with Eosinophilia and Systemic Symptoms have been reported in post-marketing experience. Discontinue therapy at first sign of a rash
• Serious angioedema and hypersensitivity, with rash, dysphagia, and bronchospasm, have been reported. Rare cases of life threatening multiorgan hypersensitivity reactions have also been reported
• Because of psychoactive and euphoric effects, modafinil has potential for abuse
• Post marketing psychiatric adverse events including mania, delusions, hallucinations and suicidal ideation have been reported
• Caution patients about driving, operating machinery, and engaging in other hazardous tasks requiring mental and physical alertness
• It is not recommended for use in patients with history of left ventricular hypertrophy or mitral valve prolapse who have experienced the mitral valve prolapse syndrome when receiving CNS stimulants. Use with caution in patients with recent history of myocardial infarction or unstable angina
• Monitor for rash, or emergence or exacerbation of psychiatric symptoms. Consider monitoring BP

• HEMA: Eosinophilia, agranulocytosis
• DERM: Dry skin, sweating, herpes simplex, serious rash
• MUSC: Joint pain, neck pain
• MISC: Infection, flu-like syndrome, chest pain, chills
