Adult Dosing
Allergic rhinitis
- 2.5-5 mg PO daily in the evening
Uncomplicated skin manifestations of chronic idiopathic urticaria
- 2.5-5 mg PO daily in the evening
Pediatric Dosing
Perennial allergic rhinitis
- 6 months-5 yrs: 1.25 mg PO daily in the evening (1/2 tsp oral solution); do not exceed recommended dose
- 6-11 yrs: 2.5 mg PO daily in the evening (1/2 tablet or 1 tsp oral solution); do not exceed recommended dose
- >12 yrs: 2.5- 5 mg PO daily in the evening
Seasonal allergic rhinitis
- 2-5 yrs: 1.25 mg PO daily in the evening (1/2 tsp oral solution); do not exceed recommended dose

Renal Dose Adjustment (Based on CrCl)
- Mild renal impairment (50-80 mL/min): 2.5 mg PO daily
- Moderate renal impairment (30-50 mL/min): 2.5 mg PO qod
- Severe renal impairment (10-30 mL/min): 2.5 mg PO twice a week
- End-stage renal disease (<10 mL/min): Contraindicated
- Hemodialysis: Contraindicated
Hepatic Dose Adjustment
- Hepatic impairment: No dose adjustments
- Caution patients against engaging in hazardous activities requiring complete mental alertness, and motor coordination such as operating machinery or driving a motor vehicle
- Advise patients to avoid concurrent use with alcohol or other CNS depressants as this will cause additional reductions in alertness and additional impairment of CNS performance may occur
- Urinary Retention has been reported with postmarketing use of levocetirizine. Use cautiously in patients with medical conditions that would predispose them to urinary retention. Consider discontinuation of therapy if urinary retention occurs

Pregnancy Category:B
Breastfeeding: Drowsiness and other effects in the infant or decrease in milk supply have occurred with using higher doses or long term use particularly in combination with a sympathomimetic such as pseudoephedrine or before lactation. The british society for allergy and clinical immunology recommends cetirizine, the racemic form of the drug, at its lower dose as a preferred choice if an antihistamine is essential during breastfeeding.
