Hyperphosphatemia in ESRD

• 1334-2668 mg PO qd divided bid with meal. Adjust dose to keep serum phosphate < 6 mg/dL as long as hypercalcemia does not develop

• Always start with low dose and should not be increased without careful monitoring of serum calcium
• Avoid use of concurrent calcium supplements as it may develop hypercalcemia in patients with ESRD
• Monitor for progressive hypercalcemia due to overdose of calcium acetate which may be severe and may require emergency measures
• Hypercalcemia due to overdosage may precipitate cardiac arrhythmias. Avoid use in patients on digitalis
• Monitor serum calcium level twice weekly during the early dose adjustment period as chronic hypercalcemia may occur leading to vascular calcification and other soft-tissue calcification
• Reduce dosage or discontinue therapy immediately depending on the severity of hypercalcemia
• Do not allow serum calcium-phosphate product (CaXP) product to exceed 66
• Radiographic evaluation of suspect anatomical region should be performed in early detection of soft tissue calcification
