- Central nervous system stimulation with convulsions or cardiovascular collapse with accompanying hypotension can occur with sympathomimetic amines
- Administer lower initial dose of patients with decreased renal function because they have reduced elimination of cetirizine and pseudoephedrine
- Patients should be warned against driving a car or operating potentially dangerous machinery after taking cetirizine/pseudoephedrine as it can cause somnolence in some patients
- Cetirizine/pseudoephedrine should not be used with alcohol or other CNS depressants, as it can cause additional reductions in alertness and additional impairment of CNS performance
Cautions: Use cautiously in
- Renal impairment
- Hepatic impairment
- Hypertension
- Diabetes mellitus
- Ischemic heart disease
- Increased intraocular pressure
- Hyperthyroidism
- Prostatic hypertrophy
- Concomitant use of CNS depressant

Breastfeeding: Prolong use or high dose of cetirizine may cause drowsiness and other effects in the infant or decrease the milk supply if used before lactation is well established or in combination with a sympathomimetic such as pseudoephedrine. The British Society for Allergy and Clinical Immunology recommends lower dose of cetirizine if an antihistamine is required during breastfeeding. Small amounts of pseudoephedrine in breastmilk are unlikely to harm the nursing infant, however may cause irritability occasionally. Acute decrease in milk production can occur with a single dose of pseudoephedrine and repeated doses may interfere with lactation. Therefore mothers of newborns whose lactation is not yet well established or having difficulties producing sufficient milk should not receive pseudoephedrine. This information is based upon LactMed database (available at http://toxnet.nlm.nih.gov/cgi-bin/sis/htmlgen?LACT last accessed 23 February 2011). As per manufacturer's data as both cetirizine and pseudoephedrine are excreted in milk, use of cetirizine/pseudoephedrine is not recommended in nursing mothers.
