Adult Dosing
Erosive esophagitis associated with (GERD)
- 40 mg PO daily x 8 wks
- Additional 8 wks of treatment can be considered in individual who have not healed after 8 wks
Maintenance of healing of erosive esophagitis
Pathological hypersecretory conditions, including Zollinger-Ellison syndrome
- 40 mg PO bid, as long as clinically indicated
- Max: 240 mg/day
Notes:- Administer ETABS with or without food and do not cut crush or chew tabs
- Administer ESUSP 30 minutes prior to meal via oral administration in apple juice or applesauce
Pediatric Dosing
Erosive Esophagitis Associated With Gastroesophageal Reflux Disease (>5 yrs of age)
- Children 15 kg-<40 kg: 20 mg PO daily x 8 wks
- Children 40 kg: 40 mg PO daily x 8 wks
Notes:- Administer ETABS with or without food and do not cut crush or chew tabs
- Administer ESUSP 30 minutes prior to meal via oral administration in apple juice or applesauce

- Symptomatic relief with pantoprazole therapy does not exclude the presence of gastric malignancy
- Long term treatment with pantoprazole can cause atrophic gastritis especially in patients with H. pylori infection
- Long term treatment with acid-suppressing medications can cause malabsorption of cyanocobalamin (Vitamin B-12) caused by hypo- or achlorhydria leading to cyanocobalamin deficiency
- Increased risk for osteoporosis-related fractures of the hip, wrist, or spine has been reported with proton pump inhibitor (PPI) therapy. Administer the lowest dose and shortest duration of PPI therapy and manage the patient at risk of osteoporosis-related fractures according to established treatment guidelines
- Rare types of gastrointestinal tumors have been observed in long-term rodent studies of pantoprazole. The relevance of these findings to tumor development in humans is unknown
- Proton pump inhibitors can cause false positive urine screening tests for tetrahydrocannabinol (THC), consider an alternative confirmatory method to verify positive results
- Concomitant use with nelfinavir or atazanavir not recommended
- Use of PPIs with high dose methotrexate may increase and prolong serum levels of methotrexate and its metabolite leading to methotrexate toxicities. Temporary withdrawal of the PPI must be consider in patients who are on high-dose of methotrexate
- PPI therapy including pantoprazole may increase risk of Clostridium difficile associated diarrhea, particularly in hospitalized patients

- Advise patients not to split, crushed, or chew tabs
- Advise patients that delayed-release oral suspension should only be administered in apple juice or applesauce, not in water, other liquids, or foods
Pregnancy Category:B
Breastfeeding: Limited data indicates low level of pantoprazole is secreted in milk following the maternal dose of 40 mg daily and is not expected to cause any adverse effects in breastfed infants. This information is based upon LactMed database (available at http://toxnet.nlm.nih.gov/cgi-bin/sis/htmlgen?LACT last accessed 22 April 2011). As per manufacturer's data pantoprazole is excreted in human milk. Because of the potential for serious adverse reactions in nursing infants and based on the potential for tumorigenicity shown for pantoprazole in rodent carcinogenicity studies, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the benefit of the drug to the mother.
