Adult Dosing
Intermittent claudication, symptomatic treatment
- 100 mg PO bid
- 50 mg PO bid if taking CYP3A4 inhibitors (like ketoconazole, itraconazole, erythromycin, diltiazem) or CYP2C19 inhibitors (omeprazole)
- Note: Give 30 mins before or 2 hrs after breakfast and dinner; beneficial effect may take up to 12 weeks
Pediatric Dosing
- Safety and efficacy in pediatric patients have not been established
Renal Dose Adjustment (Based on CrCl)
- < 25 mL/min: Use with caution, dose adjustments not defined
Hepatic Dose Adjustment
- Mild hepatic impairment: No dose adjustments
- Moderate-severe hepatic impairment: Use with caution, dose adjustments not defined
- Cilastozol and its metabolites are inhibitors of phosphodiesterase III; risk of decreased survival compared to placebo in patients with CHF [U.S Black Box warnings]
- Rare cases of thrombocytopenia or leukopenia progressing to agranulocytosis have occurred, however, agranulocytosis was reversible on immediate discontinuation of cilostazol
- Limited information on safety and efficacy of concurrent use of cilostazol and clopidogrel; check bleeding times during coadministration
- Avoid grapefruit juice
- Monitor patient for signs and symptoms of bleeding, especially those concurrently on other anticoagulants
- Assess patient for adverse cardiac events and for signs of CHF
Cautions: Use cautiously in
- Severe renal impairment
- Moderate to severe hepatic impairment
- Cardiovascular disease
- Concomitant use of other antiplatelet agents
- Concomitant use of CYP3A4/CYP2C19 inhibitors (dose reduction recommended)
- Thrombocytopenia
- Risk of bleeding from surgery or pathological process
