Liebowitz Social Anxiety Scale

The Liebowitz Social Anxiety Scale (LSAS) is a 24-item clinician administered or self-rating scale for measuring social anxiety (phobia).

Of the 24 items, 13 relate to performance anxiety and 11 relate to social situations. Each of these 24 items is rated separately for Fear/Anxiety and Avoidance.

For Fear/Anxiety:

  • Not at all = 0 pt
  • Mild = 1 pt
  • Moderate = 2 pts
  • Severe = 3 pts

For Avoidance:

  • Never = 0 pt
  • Occasionally = 1 pt
  • Often = 2 pts
  • Usually = 3 pts

The clinician/patient should answer the questions based on their experience in the past week. All items should be rated for both Fear AND Avoidance.

  • Performance Anxiety Avoidance
    • Avoidance score of questions 1, 2, 3, 4, 6, 8, 9, 13, 14, 16, 17, 20, 21
    • Score range 0-39 pts
  • Social Situation Fear/Anxiety
    • Fear score of questions 5, 7, 10, 11, 12, 15, 18, 19, 22, 23, 24
    • Score range 0-33 pts
  • Social Situation Avoidance
    • Avoidance score of questions 5, 7, 10, 11, 12, 15, 18, 19, 22, 23, 24
    • Score range 0-33 pts

Total Score (range 0-144 pts)

  • 55-65: Moderate social anxiety/phobia
  • 65-80: Marked social anxiety/phobia
  • 80-95: Severe social anxiety/phobia
  • >95: Very severe social anxiety/phobia
