Ureter injuries are graded I-V with increasing severity having higher grading.
- Grade I: Ureteral hematoma or contusion without devascularization
- Grade II: Ureteral laceration (<50% transection)
- Grade III: Ureteral laceration (>50% transection)
- Grade IV: Ureteral laceration with complete transection with <=2 cm devascularization
- Grade V: Ureteral laceration with complete transection with >2 cm devascularization
The grading is increased by 1 grade for multiple injuries or bilateral injuries up to grade III.
