• The genogram (see example) should include a minimum of the patient’s grandparents, parents, aunts, uncles, and siblings (and children and grandchildren, if applicable).
• The age of each family member or age at the time of death is noted inside each individual’s symbol.
• Date of genogram construction is noted.
• Abbreviations should be defined.
