Time-out is a formal process of verification performed in-person and simultaneously by Pt and health care team to verify correct Pt, correct surgery, correct site, and correct side (if applicable).
- Correct Pt: With pre-op and circulating nurses both present, Pt asked to state their full name as printed on wristband and at least one other identifier (e.g., DOB, address, phone number).
- Correct Surgery: Pt asked to state correct surgery.
- Correct Site/Side: Pt asked to state site, and side if applicable.
- Correct Consent: Pt asked to verify information and signature on consent is correct.
- Allergies Verified: Pt asked to verify known allergies or sensitivities.
- Orders Completed: Pre-op orders verified as completed.
- Antibiotics: Antibiotics (if ordered) verified as given or available.
- This time-out process is completed again by surgical team in the OR immediately prior to induction and incision.
