First responsibility of preoperative (pre-op) nurse is to review information on wristband with Pt and have Pt verify all information.

• Identification wristband is placed on Pt upon admission to facility.
• Pt/guardian must verify with nurse that all information is correct.
• Wristband must be verified before Pt moved into surgical prep area.
• Wristband(s) must match information on surgery schedule.
• Allergy wristband placed if allergies present.
• Limb alert wristband placed on affected arm if applicable.
• Blood bank wristband placed (if type-and-crossmatch done).
• Minors: Legal guardians have guardian wristbands in place.
