Pt Privacy: Verify Pt has designated a contact person on a release of information (ROI) form. This allows surgeon and staff to communicate pertinent, necessary information to caregiver—refer to facility guidelines and the Health Insurance Portability and Accountability Act (HIPAA) to ensure protection of Pt privacy.

• Verify presence of admitting and pre-op orders.
• Review surgical and medical history including prescription, OTC, herbal, and other alternative therapies (see table below).
• Perform initial safety check (time-out) where Pt verifies:
• All information on wristband as correct.
• Name of surgeon and surgery being performed.
• Correct surgical site and side (if applicable).
• Consent matches H&P and PARQ documented.
• Verify Pt and a witness (if applicable) signed informed consent.
• Review Pt education regarding entire surgical process.
• Review current health status and baseline VS.
• Locate and record all implants and assistive devices.
