Postpone therapy until ANC >1,500/mcL or platelets >100,000/mcL; reduce dose by 50 mg/M²/day for subsequent cycle

Based on lowest counts at either Day 22 or Day 29

ANC < 1,000/mcL or platelets < 50,000/mcL
