Therapeutic Classification: antipsychotics, mood stabilizers

Pharmacologic Classification: benzisoxazoles

Indications ⬆ ⬇

BEERS REMS

Action ⬆ ⬇

  • An atypical antipsychotic that may act by antagonizing dopamine and serotonin receptor in the CNS.
Therapeutic effects:
  • Decreased symptoms of schizophrenia or bipolar mania.

Pharmacokinetics ⬆ ⬇

Absorption: Extent of absorption following oral administration unknown.

Distribution: Extensively distributed to tissues.

Metabolism/Excretion: Milsaperidone undergoes oxidation to form iloperidone P95. Iloperidone is converted to milsaperidone. Iloperidone is extensively metabolized by the liver, primarily by the CYP3A4 and CYP2D6 isoenzymes. The CYP2D6 enzyme system exhibits genetic polymorphism (7–10% of White patients and 3–8% of Black patients are considered poor metabolizers [PMs]). 58% excreted in urine as metabolites in normal metabolizers and 45% in PMs; 20% eliminated in feces in normal metabolizers and 22% in PMs.

Half-Life: Normal metabolizers: milsaperidone: 26 hr; iloperidone: 18 hr; P95: 23 hr; PMs: milsaperidone: 37 hr; iloperidone: 33 hr; P95: 31 hr.
