MIR-ve-TUX-i-mab SOE-rav-TAN-seen

Classifications ⬆ ⬇

Therapeutic Classification: antineoplastics

Pharmacologic Classification: drug-antibody conjugates, antimicrotubulars, monoclonal antibodies

Indications ⬆ ⬇

High Alert

Action ⬆ ⬇

  • The monoclonal antibody portion (mirvetuximab soravtansine) attaches to the folate receptor alpha on the surface of tumor cells. A microtubule inhibitor (DM4) is linked to the antibody portion. Binding of the antibody-drug conjugate to the folate receptor alpha allows internalization of the complex into the tumor cells. Once internalized, the DM4 is released and disrupts the microtubule network within the cell, resulting in tumor cell death.
Therapeutic effects:
  • Decreased progression of epithelial ovarian, fallopian tube, or primary peritoneal cancer.

Pharmacokinetics ⬆ ⬇

Absorption: IV administration results in complete bioavailability.

Distribution: Minimally distributed to tissues.

Protein Binding: >99%.

Metabolism/Excretion: The monoclonal antibody portion (mirvetuximab soravtansine) is broken down into small peptides by catabolic pathways. Unconjugated DM4 and its metabolite, S-methyl-DM4, are metabolized by the liver via the CYP3A4 isoenzyme. S-methyl DM4 and DM4-sulfo-SPDB-lysine are the main metabolites detected in urine.

Half-Life: Mirvetuximab soravtansine: 4.8 days; Unconjugated DM4: 2.8 days; S-methyl-DM4: 5 days.
