Because of the intensive surgical intervention needed during a pancreaticoduodenectomy, the patient is usually admitted to the surgical critical care unit for recovery. Preventive measures are required to avoid complications such as pancreatitis, hepatic failure, infection, fistulas (pancreatic, gastric, and biliary), renal failure, ARDS, pulmonary embolism, thrombophlebitis, or heart failure. Upon arrival to the PACU or ICU, the patient is placed in the semi-Fowler’s position to reduce tension on the suture line and anastomosis site. This position also allows the lungs to expand in order to avoid pulmonary complications. Because of the extensive duration of the procedure, maintaining an adequate fluid and electrolyte balance is primordial. In addition, glucose levels should be monitored frequently to prevent hypo/hyperglycemia due to the stress and manipulation of the pancreas (Ignatavicius & Workman, 2015).
