Frequency of vital signs is dependent on the patient’s condition and unit policies. Most patients placed in the observation category have received Phase I level of care, so they have stabilized to advance their vital signs from a range of 1 to 4 hours to every 4 to 6 hours, depending on their acuity and current condition. Minimum vital signs include temperature, blood pressure, pulse, respirations, pulse oximetry, and pain assessment.
