Assessment of the newborn includes airway patency, chest wall movement (respiratory effort), respiratory rate, breathing pattern, breath sounds, heart rate and rhythm, abdomen (soft, nondistended), umbilical stump (number of vessels), skin color, genitalia, alertness, activity, tone and movement of extremities, and anomalies or birth trauma (nerve injuries) (Raab & Kelly, 2013).
