A. Characteristics
- Febrile viral infection
- Progression to severe noncardiogenic pulmonary edema
- Disease in USA caused by two "new" hantaviruses
- Sin Nombre Virus (SNV, transmitted by Deer Mouse) [1]
- Black Creek Canal Virus (BCCV, transmitted by cotton rats) [2]
- Old world hantaviruses include Puumala, Seoul, and Hantaan
- Transmitted to humans from rodent excrement only, not insect-borne [3]
- Related to hemorrhagic fever viruses
B. Symptoms and Signs
- Fever
- Myalgias, chills, abdominal pain, nausea
- Shortness of breath
- Progressive to severe hypoxemia
- Organ Dysfunction
- Sepsis Syndrome (including disseminated intravascular coagulopathy, DIC)
- Acute Renal Failure
- Rhabdomyolysis
- Respiratory Failure
C. Laboratory
- DIC: reduced platelets, increased PT and PTT, fibrin split products
- Severe A-a gradient
- Acute Renal Failure - BUN and creatinine elevations
- Hemolysis and/or Rhabdomyolysis
- Leukocytosis or Leukocytopenia with immature forms
- Antisera are available
D. Treatment
- Mainly supportive
- May require ventilatory support
- No specific therapy currently known
