- This test measures the concentration of the hormone gastrin in the blood
- Gastrin is a polypeptide hormone synthesized and stored in the antral G cells in stomach and to lesser degree in the islets of Langerhans in the pancrease
- The function of gastrin includes:
- Facilitate food digestion by triggering gastric acid secretion from parietal cells in the stomach
- Stimulate release of
- Pancreatic enzymes from acinar cells
- Pepsinogen from chief cells
- Insulin from the pancreas
- Intrinsic factor secretion
- Increase gastric and intestinal motility
- Stimulate bile flow
- Induce gastric blood flow
- Promote gastric mucosal growth
Clinical 
- The clinical utility testing gastrin levels include:
- Aids in the diagnosis of gastrinoma, or gastrin-secreting tumors as in Zollinger-Ellison syndrome
- Aids in the differential diagnosis of gastric and duodenal ulcers
- Assists in the diagnosis of pernicious anemia and G cell hyperplasia
- Gastrin is the first-line test for determining whether a patient with active ulcer disease has a gastrinoma
- Gastrin is secreted in response to:
- Vagal stimulation
- Presence of food, alcohol, or calcium in the stomach
- Alkalinity of gastric secretions
- Gastrin-releasing peptide (GRP)
- Gastrin is inhibited in response to (negative feedback)
- Presence of acid (pH <3) in the stomach
- Somatostatin from D cells of stomach
- Gastric inhibitory peptide (GIP)
- Secretin
- Glucagon
- Calcitonin
- Zollinger-Ellison syndrome occurs sporadically in 75% cases and 25% cases are associated with multiple endocrine neoplasia Type 1 (MEN 1)
- Hypergastrinemia may clinically manifest as:

- Gastroesophageal reflux symptoms
- Gastrointestinal bleeding
