A. Characteristics
- Elevation of White blood count (WBC) > 100K/µL (hyperleukocytosis)
- Symptoms of vascular interruption
- Stroke
- Renal Failure
- Myocardial Infarction
- Skin Disease - purpura, others
- Pulmonary Failure
- This is a medical emergency
B. Occurrence
- AML
- CML with blast crisis
- ALL
- Uncommon in CLL (very small cells)
C. Treatment
- Intravenous Hydration
- Allopurinol
- Reduces uric acid production
- Great deal of uric acid released with spontaneous or drug induced tumor cell lysis
- Hydroxyurea - rapid kill of tumor cells
- Cranial Irradiation - may prevent strokes; efficacy still controversial
- Specific therapy for underlying disease
- AML: Daunorubicin + Ara C then High Dose AraC (HiDAc)
- ALL: glucocorticoids, alkylating agents, CNS therapy
- Leukopheresis is marginally effective, used in life-threatening situations
- Central nervous system symptoms
- Pulmonary failure
