Multiple myeloma criteria

Multiple myeloma is a cancer of plasma cells and is a progressive hematologic disease that currently has no cure, but is treatable. Incidence is 5-6/100,000/year with a male and African American predominance. Age is a major risk factor, with just 1% of cases occurring under age 40 years and the average age at diagnosis being 68 years.

The diagnostic criteria of Durie and Salmon require either:

  • 1 Major Criteria and 1 Minor Criteria
  • 0 Major Criteria and 3 Minor Criteria

Major Diagnostic Criteria

  • Plasmacytoma on tissue biopsy
  • Bone marrow plasmacytosis of >30%
  • M Protein (IgG >3.5 g/dL; IgA >2.0 g/dL)
  • Urinary kappa or lambda chain excretion of >1g/24 hours (Amyloidosis must be absent)

Minor Diagnostic Criteria

  • Marrow plasmacytosis of 10-30%
  • Lytic bone lesions
  • Monoclonal protein present (But in lesser amounts than required in major criteria)
  • Hypoglobulinemia of normal proteins (IgM <500 mg/L, IgA <1 g/L or IgG <6 g/L)
