Moist ulcer (non-healable)

Cleanse with sterile normal salline or water
Pat dry to remove excessive moisture
If slough or moist necrotic tissue present, apply antiseptic/antimicrobial agent to reduce bacterial load (e.g., silver containing product)
Cover with protective non-stick dry dressing
Keep surrounding tissue protected with moisture barrier (do not apply to wound)
Change dressing as needed to minimize bacterial load

Dry eschar covered wound (non-healable)

Cleanse eschar with normal saline or water
Pat dry to remove excessive moisture
Apply antiseptic solution to keep eschar dry and control bacterial load (e.g., silver containing product)
Cover with protective non-stick dry dressing
Keep surrounding tissue protected with moisturizer if skin is dry—do not apply to eschar
Change dressing as needed to minimize eschar disturbance
