If the distance between the R-R intervals is the same throughout the ECG strip, the rhythm is regular. If there are less than three small boxes (less than 0.12 sec) of variation between the shortest and longest R-R interval the rhythm may still be called regular. At fast heart rates over 100 bpm, the three small box leeway in regularity does not apply.
