Ziv-aflibercept is available as a 25-mg/mL concentrate for injection in single-use vials containing 100 or 200 mg of the drug.²⁸⁷⁷ Each vial also contains polysorbate 20 0.1%, sodium chloride 100 mM, sodium citrate 5 mM, sodium phosphate 5 mM, and sucrose 20% in water for injection.²⁸⁷⁷ The concentrate must be diluted before administration.²⁸⁷⁷

Zaltrap

Ziv-aflibercept is administered by intravenous infusion over one hour following dilution in a compatible diluent to a concentration of 0.6 to 8 mg/mL.²⁸⁷⁷ The diluted drug should be administered through a 0.2- or 0.22-µm inline polyethersulfone filter.²⁸⁷⁷; ²⁸⁷⁸ Ziv-aflibercept should not be administered as a bolus or push intravenous injection.²⁸⁷⁷

Ziv-aflibercept concentrate is a clear, colorless to pale yellow solution.²⁸⁷⁷ Intact vials should be stored under refrigeration and kept in the original outer carton to protect from light.²⁸⁷⁷

Visually inspect ziv-aflibercept before use.²⁸⁷⁷ Discard if the solution is cloudy, contains particles, or is discolored.²⁸⁷⁷

Single-use vials of ziv-aflibercept do not contain any preservatives.²⁸⁷⁷ After the appropriate dose has been withdrawn from the vial upon initial puncture, the vial should not be re-entered, and any unused portion should be discarded.²⁸⁷⁷

Diluted solutions of ziv-aflibercept should be prepared in infusion containers composed of either PVC (containing either diethylhexyl phthalate [DEHP] or trioctyl trimellitate [TOTM]) or polyolefin (e.g., polypropylene, polyethylene).²⁸⁷⁷; ²⁸⁷⁸ Ziv-aflibercept should not be prepared in infusion containers composed of ethylene vinyl acetate (EVA).²⁸⁷⁸

Diluted solutions of ziv-aflibercept should be used immediately.²⁸⁷⁸ Admixtures not used immediately should be refrigerated for up to 4 hours.²⁸⁷⁷ Any unused portion remaining in the infusion bag following administration should be discarded.²⁸⁷⁷

Light Effects

Ziv-aflibercept concentrate is light sensitive.²⁸⁷⁸ Vials of the concentrate should be stored in the original outer carton to protect from light.²⁸⁷⁷ The manufacturer does not specify a need for protecting the diluted solution from light.²⁸⁷⁸

Sorption

No substantial changes in the quality or potency of the drug were noted when diluted solutions of ziv-aflibercept (0.6 to 8 mg/mL) were tested with infusion sets composed of DEHP-plasticized PVC, TOTM-plasticized PVC, polyethylene-lined PVC, or polypropylene.²⁸⁷⁷; ²⁸⁷⁸ Infusion sets composed of polyurethane also were found to be compatible.²⁸⁷⁷; ²⁸⁷⁸

Filtration

For ziv-aflibercept administration, the manufacturer recommends the use of either a 0.2- or 0.22-µm inline polyethersulfone filter.²⁸⁷⁸ Filters composed of polyvinylidene fluoride or nylon were noted to facilitate adsorption during stability testing²⁸⁷⁸ and should not be used.²⁸⁷⁷
