Bivalirudin is available as a white lyophilized powder or cake.3287 Each single-dose vial contains bivalirudin 250 mg with mannitol 125 mg and sodium hydroxide to adjust the pH.3287 Vials should be reconstituted with 5 mL of sterile water for injection and swirled to dissolve, yielding a 50-mg/mL solution.3287 The reconstituted solution should be further diluted to a concentration of 0.5 or 5 mg/mL prior to administration.3287
