The fixed combination of aztreonam-avibactam sodium is available as a lyophilized powder in single-dose vials containing 2 g (aztreonam 1.5 g plus avibactam 0.5 g as the sodium salt).4071 Each vial also contains L-arginine 1170 mg/vial.4071
Reconstitute each vial with 10 mL of sterile water for injection.4071 The vial contents should be mixed gently to ensure complete dissolution to yield approximately 12 mL of solution containing approximately 127 mg of aztreonam plus 42.3 mg of avibactam in each mL.4071 The reconstituted solution must be diluted in a compatible infusion solution (sodium chloride 0.9%; dextrose 5%; or Ringer's injection, lactated) in a 50- to 250-mL infusion bag to achieve a concentration of aztreonam 2.7 to 40 mg/mL and avibactam 0.9 to 13.3 mg/mL prior to infusion.4071
The diluted solution for infusion should be mixed gently to ensure complete dissolution.4071 Visually inspect for particulate matter and discoloration prior to administration.4071
The manufacturer’s instructions for preparing various doses of aztreonam-avibactam are as follows:4071
For a 2.67-g loading dose (aztreonam 2 g plus avibactam 0.67 g), withdraw 15.7 mL of reconstituted solution (requires 2 vials) and dilute in a compatible infusion solution; discard unused portion of vials.
For a 1.8-g loading dose (aztreonam 1.35 g plus avibactam 0.45 g), withdraw 10.6 mL of reconstituted solution from the vial and dilute in a compatible infusion solution; discard unused portion.
For a 1.33-g loading dose (aztreonam 1 g plus avibactam 0.33 g), withdraw 7.9 mL of reconstituted solution from the vial and dilute in a compatible infusion solution; discard unused portion.
For a 2-g maintenance dose (aztreonam 1.5 g plus avibactam 0.5 g), withdraw entire contents of 1 vial (approximately 12 mL) and dilute in a compatible infusion solution.
For a 1-g maintenance dose (aztreonam 0.75 g plus avibactam 0.25 g), withdraw 5.9 mL of reconstituted solution from the vial and dilute in a compatible infusion solution; discard unused portion.
For a 0.9-g maintenance dose (aztreonam 0.675 g plus avibactam 0.225 g), withdraw 5.3 mL of reconstituted solution from the vial and dilute in a compatible infusion solution; discard unused portion.
