Etiology:

1. acquired (usually gunshot wound)
2. congenital AVF

Location: lower extremity most frequent

• soft-tissue mass
• presence of large vessels
• phleboliths (DDx: long-standing varicosity)
• accelerated bone growth
• cortical osteolytic defect (= large-vessel pathway into medulla)
• increased bone density
