Suggested NOC Outcomes

• Conduct environmental assessment. Patient should be able to identify risks for falls and ability to manage care outside the hospital.
• Educate on importance of regular exercise, which can improve mobility, improve gait, decrease incidence of falls, increase muscle strength.
• Implement interdisciplinary treatment plan, which can improve physical and psychological function, improve patient satisfaction, decrease need for hospitalization.
• Provide patient with ample time to complete ADLs to increase ability to complete ADLs.
• Provide physical and occupational therapy as needed to improve functional impairment.
• Provide socialization opportunities. Isolation can exacerbate depression.
• Provide stimulating activities to keep cognitive mind engaged.
• Provide supplemental nutritional drinks to provide extra calories and energy intake.
