Rubber Band Ligation for Haemorrhoids Using a Suction Instrument

A rubber band Rubber Band Ligation Compared with other Modes of Therapy for Symptomatic Hemorrhoids is inserted on the instrument by using a cone. The air canal of the instrument is closed by finger to apply suction. The function of the instrument is first demonstrated by placing a rubber band on a paraffin film filled with ultrasound gel. After applying the suction, wait until sufficient negative pressure has developed into the cup of the instrument, filling the cup with a bulge of rectal mucosa and veins inside. At the start of the procedure a proctoscope is inserted into the rectum and pulled back until the border between rectal mucosa and anal canal (linea dentata) is visible. Place the suction cup of the instrument above linea dentata (where there are no nerve endings for pain) at the base of the haemorrhoid. Tilt the instrument towards the rectal wall on the side of the haemorrhoid, apply suction, wait until sufficient negative pressure has developed, and pull the trigger to position the rubber band. Stop the suction and remove the instrument together with the proctoscope.
