A 69-year-old woman had noted a lump in her neck. The left lobe of the thyroid gland was enlarged. During swallowing the thyroid gland moves with the laryngx. Ultrasonography shows a large, multilocular cyst with some soft tissue. The first image is transversal, showing the carotid artery on the left side of the thyroid gland. The second image is longitudinal. The patient was referred to needle biopsy and consultation by a surgeon.
