Type of Acetabular Cup for Total Hip Replacement

Threaded acetabular cups may have a higher failure rate than cemented or porous in-growth cups. Level of evidence: "C"

A systematic review 1 including 95 studies: 17 for threaded cup (1 148 cases), 22 for the porous in-growth cup (1 979 cases), and 56 for the cemented cup (10 230 cases), was abstracted in DARE.

The threaded cup group, compared with the cemented and porous control group, had a significantly higher rate of revision (3.58% compared with 1.61% and 1.44%, respectively). The same was true for pain score (15.10%, 4.53% and 3.01%), and migrations (8.85%, 1.48% and 0.64%).

Comment: The quality of evidence is downgraded by indirectness (differences in studied patients) and study quality (the data are mainly from patient series and non-comparative studies).
