Olanzapine for Mania in Bipolar Type 1 Disorder

A systematic review found that olanzapine significantly increased the proportion of people who responded over 3-4 weeks compared with placebo (2 RCTs, proportion who failed to respond by 50% reduction in mania score; 56/125 (45%) with olanzapine vs 89/129 (69%) with placebo, RR 0.64, 95% CI 0.52 to 0.81). According to one RCT, adding olanzapine to lithium or valproate increased the number of responders (80/229 (35%) with olanzapine vs 64/115 (56%) with placebo, RR 0.63, 95% CI 0.49 to 0.80). Olanzapine caused significantly more weight gain than placebo. Clinical Evidence category: Beneficial.
